Performer/model Release form
Hey everyone,
Hoping someone could help me. Does anyone have a copy of a model release form that they use for burlesque dancers?
If someone does have a copy can they forward a copy to me at alm@wha.bc.ca

Do make sure you get a release form that's compatible with Canadian law - I'm afraid I can only direct to UK resources

ones that i've seen modeling tend to be
- giving permission for photographer/ videographer to use pics/ footage in a way they see fit
- signing over the copyright to the tog/vtog
- agreeing to publication
- agreeing to non publication
- agreement to the amount paid
- conformation of personal details, DoB etc
Can't think of anything else right now, other thing is to try a site like How To Become A Model - Modeling Portfolios - Fashion, Glamour and Photographic Models think you can use it as a guest, just ask in one of teh seeking advice forums for a general template that you can modify.

Are you talking about a release for photos shoots or for live performance shots? The best advice for shoots I've come across is here...
Damon Allen Davison - Photographer agreements for models
For live performance shots I don't really think there is any standard form. I clear all my work through the events organisers (I wouldn't dream of going to a show and taking photos without the organisers permission as I feel the performers would be well within their rights to object to any unauthorised photographers) and make sure that performers get complete access to my work as I send out CDs of high res.
